WellSpace Health at Salvation Army
WellSpace Health operates an 18-bed Interim Care Program (ICP) and 5-bed ICP+ program within the Salvation Army. Working with the four hospital systems (Sutter, Mercy, Kaiser, UC Davis Medical Center), ICP and ICP+ bridge the gap for homeless patients who are well enough to be discharged from the hospital but not able to meet their medical needs while homeless. This site is also a Federally Qualified Health Center satellite medical clinic to the J Street Community Health Center.
The program is only available via hospital discharge planner.
