King Jr. Village

As a Federally Qualified Health Center satellite medical clinic to the J Street Community Health Center, The Effort at Martin Luther King Jr. Village clinic provides onsite Primary Care and Behavioral Health to residents of this permanent supportive housing program. 50 of the 80 cottages are designated to residents who receive services from The Effort’s site-based Supportive Services Team.; Many of these clients come via the “T3” program. T3 (Triage-Transport-Treat) offers wrap-around community services for persons who are multiple users of Emergency Departments for non-urgent care and deserve a supportive team capable of connecting them to a healthcare home and social services.
In this way, The Effort is the ‘relief valve’ to our community’s over-crowded ER’s. By transferring some patients into the appropriate care of The Effort we are able to cut down on Emergency Room waiting times while assessing and treating patients’ health, mental health, and/or addiction problems. This method has been shown to reduce participant’s ER visit rate by 65% thereby driving down costs to hospitals and the community, while providing treatment for core issues.
