J Street Community Health Center
The Effort has operated a safety net clinic in midtown Sacramento since 1969, and is designated as a Federally Qualified Health Center (FQHC). As The Effort’s flagship, The J Street Community Health Center offers primary care, addiction and mental health treatment, and is the agency’s administrative headquarters. Clients and patients may receive ‘suites’ of services that integrate with the Primary Care Clinic and fulfill The Effort’s leadership role of treating the whole person.

At our J Street location, we provide integrated behavioral health care in the primary care clinic delivering co-occurring mental health and substance abuse services in conjunction with physical health care. This model addresses all aspects of patient care and provides a more complete treatment regimen. We have licensed therapists offering a wide range of clinical services including counseling for depression, anxiety, wellness, and family problems among others.
We have also provided out patient addiction treatment for 4 decades. As the leading addictions treatment provider in the Sacramento region, we offer cutting edge services integrated with all of The Effort’s health and mental health programs. Services at this site include out patient treatment, court-ordered drug education classes, as well as group and individual counseling. The Community Health Center is centrally located and adjacent to all Public Transit.
