Interim Care Program (ICP)
The Effort leads a collaboration of the hospital systems in Sacramento, community based organizations, and the county government--all of whom have come together to create a respite care program for patients discharging a hospital who are homeless. Partner hospitals include: Kaiser Morse and South; Mercy/CHW General, Methodist, San Juan, and Folsom; Sutter General and Memorial; and UC Davis Medical Center.
Over 600 patients have been served in this innovative program that has been replicated by other communities and has received statewide and national attention.
