Summary Statement
The Therapist is responsible for conducting assessments and providing counseling to children, adolescents and/or adults with
co-occurring disorders and medical complications. Will also provide consultation to physicians regarding mental health and/or
substance abuse issues and develop treatment plans.
Required Qualifications (Knowledge, Skills, Abilities, Education, Training, and Licensure):
- Licensed Clinical Social Worker (LCSW) - licensure is required
- Two plus years of experience providing child, adolescent and/or adult counseling
- Experience in a variety of assessments, treatment planning and counseling issues including chemical dependency, co-occurring
disorders and traumatic experiences
- Three to five years experience supervising clinical personnel desirable
- OR an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job
- Knowledge and skill working with diverse populations
- Knowledge of Medi-Cal requirements preferred
- Strong verbal and written communication skills
- Strong interpersonal skills
- Ability to abide by standards of professional ethics
- Computer literacy including word processing and e-mail
- Bi-lingual ability a plus
- Valid California Driver's License, with insurable driving record
Essential Responsibilities and Duties:
- Triage, assessment of behavioral health conditions including: mental health disorders, substance use / abuse / dependence
- Administer tools and provides indicated follow-up (PHQ-9, Generalized Anxiety Disorder 7-item scale, CAGE, etc.)
- Crisis intervention, risk management
- Group therapy for behavioral health and/or health conditions
- Psycho-education
- Works collaboratively with patient and team members to design individualized, culturally appropriate care plans
- Works collaboratively to provide clinical support to primary care patients in a chronic care model for health and/or
behavioral health conditions
- Assists patients in understanding, review, or updates of his/her care plan as well as understanding of treatment agreements
- Provide counseling to adults
- Brief focused individual therapy services
- Support physician / PA / NP
- Complete all required clinical and billing documentation
- Attend case management and other staff meetings as requested
- Provide information for reports to funding sources
- Represent the agency to other agencies or groups as assigned
- Supervise graduate students/interns as assigned
- May perform Utilization Review responsibilities
- Other duties as assigned
Physical Demands and Work Environment:
The work environment is characteristic of normal primary care office conditions. The physical demands described here are
representative of those that must be met by an employee to successfully perform the essential functions of this job. While
performing the duties of this job, the employee is frequently required to sit; use hands to finger, handle, or feel. The
employee is occasionally required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or
move up to 10 pounds. Specific vision abilities required by this job include close vision, distance vision and ability to
adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise
level in the work environment is usually quiet.
The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive
to personal health and safety for themselves. The employee may experience a number of unpleasant sensory demands associated
with the client's use of alcohol and drugs, and the lack of personal care. The employee must be ready to respond quickly and
effectively to many types of situations, including crisis situations and potentially hostile situations.
