Many people are uninsured and are unaware that they are eligible for low- to no-cost health care programs. Our knowledgeable and experienced Enrollment and Eligibility Department staff will help you understand and navigate the enrollment process of many low- to no-cost health care programs including, Family PACT, Medi-Cal, Medicare, Healthy Families, and more.   

If you are a new patient, our Enrollment and Eligibility Department is the place to start. Please call 916-737-5555 to schedule an appointment. We are here to help you.

Remember:

  • Please be sure to bring your proof income and photo I.D. to your enrollment and eligibility appointment.

  • We offer medical care on a sliding scale fee system depending on income if you do not qualify for specific programs or services.
