Sutter Hospital is partnering with The Effort in launching the T3 program to provide wrap-around community services for frequent users of the emergency department. The target population for the program will be patients who frequently go to the emergency department for needs which do not require emergency room care and who can be served more appropriately through The Effort’s primary care clinic. Their complex social, psychological, and health needs will be addressed intensively on an on-going basis. The Effort will provide medical services, mental health therapy, psychiatry, substance abuse treatment, and case management. As a result, the number of ER visits will decrease and clients will receive more appropriate health services at a lower cost. The financial savings to Sacramento County are expected to be substantial.
